I only see that there is one additional level of commenting. Why isn't it more hierarchical like on other sites?

This is a controversial design choice.
We know that plenty of people will not be happy about this, but we intentionally limit the depth of threads to keep conversations on topic.
This is not a free-for-all discussion board, it's primarily for questions and answers. As you branch a discussion off into further threads, the signal to noise ratio rapidly shrinks into oblivion.
To keep discussions on point, we felt like this was the correct design decision.
